AFTER FILLING OUT AND SAVING THE CASE INFORMATION FORM AND PRINTING IT, SUBMIT IT TO US ALONG WITH YOUR DOCUMENTS AS FOLLOWS:

2 - If you prefer, you can (scan them all and create a .zip file (if sending multiple docs). On the subject line of your email state: Documents Attached
